Samuel Perry Gilligan "Sammy" Dorian is the son of John Dorian and Kim Briggs. He was conceived on J.D. and Kim's first date even though they didn't have sex. After temporarily moving away, Kim moved back to Sacred Heart to give birth. Even though J.D. and Kim are in relationships with different partners, they have shared custody of Sammy and enjoy caring for him.
Sammy was named after his late grandfather, and J.D.'s late father Sam Dorian.

Sam was born at Sacred Heart Hospital. ("My Hard Labor") He is named Sam after his grandfather, J.D.'s father Sam Dorian (who he looks like according to a statement by his Uncle Dan), Perry presumably because it is the name of Perry Ulysses Cox and it "has a nice ring to it", and Gilligan because Turk won a bet (the bet J.D. lost is still unknown).
J.D., Elliot and Kim witnessed the birth. Although J.D. and Kim are no longer a couple and were fighting during the birth, J.D. and Dr. Cox believe that the child will be brought up right. He also tilts his head when daydreaming, much like his father. Dr. Cox trained his son, Jack, to make fun of Sam, which annoys J.D. ("My Lawyer's in Love")
Sam is raised by both J.D. and Kim. J.D. decided that he would move closer to Kim so Sammy wouldn't have a "part time dad". ("My Cuz") J.D. also took a job at a closer hospital so he would be more available. ("My Chief Concern") J.D. and Elliot have Sam on the weekends. ("Our Stuff Gets Real")

After J.D.'s final shift at Sacred Heart, his possible future is shown across a banner as a figment of his imagination. On his banner, he can see also Sam's future, where Izzy and Sam announce to Turk and J.D. that they are engaged. ("My Finale")
