"My Malpractical Decision" is the ninth episode of Scrubs' fourth season. Turk pulls a fast prank on the Janitor and doesn't like the payback he receives from him. Trouble abounds for J.D. and Dr. Cox when they feel threatened by a patient's daughter Neena Broderick (Julianna Margulies), a malpractice attorney. Elliot is assigned the task of distracting clumsy doctor Doug while Neena is around.

Dr. Kelso announces that Neena Broderick is about the enter the building, so Ted panics. Neena is the scariest medical malpractice attorney in the city, who seems to create problems where there were none. Dr. Kelso makes sure this place is shaped up, so they take out Doug as well. Dr. Cox doesn't think much of Neena, but isn't afraid of her either. Neena doesn’t think about anything Perry says. Neena's father is in the hospital, much to the surprise to J.D. and Dr. Cox. Neena wants to know what the situation is with her father. Dr. Cox says they elect not to do the procedure and Dr. Kelso confirms that they can refuse treatment on non-emergency patients. Neena finds J.D. and tells him she is just worried about her father. J.D. and Neena flirt in the cafeteria. She says her act is just part of the job, just like J.D. "playing" a nerdy doctor. J.D. goes to Perry and advices to do the procedure, but Perry knows it was Neena's doing. Doug wants to know if he should really be a doctor, but Elliot doesn't know what to say. Dr. Cox wants to know how Neena managed to persuade J.D.' and Neena admits she manipulated him.

Doug finds his talent as a pathologist

Turk got a new phone number, CALL-TURK. J.D. says it should only be CALL-TUR but he is willing to dial the extra K nonetheless. Turk knows this so he did something nice for J.D. - we see the janitor's mop broken, the water bucket replaced with blue paint and the walls having lots of hand prints so that Dr. Kelso thinks it was Janitor that did it. Mr. Corman has come back to the hospital as his tennis serve is not the same after his surgery. Mr. Corman nags on about his tennis serve so Turk tells him to get away and not even think about calling his cell phone. The Janitor finds out it was Turk who painted the walls with handprints so he tells Mr. Corman of Turk's cell phone number. He calls him at dinner.

Neena meets Mr. Corman on the way to see J.D. and she says she would do anything to help her father. Turk ponders what to do, and is given the option of asserting his boundaries and cutting off all contact with Mr. Corman (including changing his cellphone number), or filing a restraining order. Turk, being too selfish to give up CALL-TURK, ends up filing a restraining order against Mr. Corman. This motivates J.D. to do the procedure and Elliot to tell Doug he is not cut out to be a doctor. Doug is shocked, but finds his gift: Finding the cause of death down at the morgue as he has seen so many of his own patients die.

Neena's dad will be fine. She thanks Dr. Cox and also thanks J.D. by making out in front of Dr. Cox. J.D. is confused as she said she wasn't interested in him. In fact, she was interested so they kiss more. Mr. Corman is suing Turk, with Neena as his attorney so he leaves a message on J.D.'s answering machine. It turns out that she handed him her card when they met earlier. Just as J.D. is about to call Turk back, Neena asks if he is coming into the bedroom. Thus J.D. hangs up the phone, deletes the message and runs into the bedroom.

  • To hide him from Neena, J.D. and Elliot capture Doug is in a cloth sack and throw him out the window onto the parking lot. Janitor passes the sack, looks inside and carries it off. Doug's head is then mounted above Janitor's fireplace while Janitor pretends to read the newspaper.

  • J.D. helps set up a bed for a foxy nurse Amy at her apartment, but demands J.D. leaves straight afterwards and she begins to kiss her muscular and topless boyfriend.

Turk frames the Janitor for putting blue hand prints allover the hospital walls as a way to show J.D. that he's a good friend. Janitor gets revenge by giving Turks mobile number to a hypochondriac patient.

Episode Running Gags

  • Turk's cell phone number CALL-TUR, which Turk thinks is cool. He later decides that the best way to deal with Mr. Corman is to file a restraining order against him, mainly to avoid the other option, which is changing his cell phone number.
  • Lloyd the Delivery Guy is the loneliest person in the hospital, and is driven to ecstasy by any form of physical contact. J.D. has this same reaction simultaneously after he and Lloyd bump into each other.
  • Ted freezes up whenever Neena talks to him.
  • J.D. helps a "foxy nurse" set up a bed in a flashback, and then she makes out with her boyfriend. They are later seen making out in the supply closet.

  • Turk's cell phone number was actually purchased by the show's producers. When filming, cast and crew members would answer the phone when fans called. When they were not available, it is just a phone recording announcing either a new season, or DVD release.
  • The 'Cal Turk' fantasy doesn't appear to be fantasized by anyone, J.D. doesn't tilt his head and look into space as usual.
  • Dr. Cox calls J.D. a guy here, when he says "I'm gonna go ahead and leave this decision to the lonely guy".
  • The hungry patient in a coma is voiced by Mike Schwartz, who also appears as Lloyd Slawski in this episode.
