Mr. Hoffner was caught in an argument between Dr. Cox and J.D. when J.D. did not offer him a surgical consult. After asking Turk to agree with whatever he thought, Turk changes his mind as the surgical consult and recommends surgery. After thinking it over and talking to other people, such as the Janitor, Mr. Hoffner chooses surgery. ("My Way or the Highway")
He returns to have his gallbladder removed, and constantly asks Dr. Cox if he really needs his gallbladder, annoying him. ("My Drive-By")

- Mr. Hoffner has not had a girlfriend for the last 20 years.
- Mr. Hoffner prefers chicken over meat.
- Mr. Hoffner also seems to have an unusually strong attachment to his gallbladder, so strong that "the only way that man could be more attached, is if he in fact rented a motel room and have sex with it", as Dr. Cox so elegantly put it.
