Mr. Bober has several symptoms, and causes his doctor to do a lot of work. J.D. pawns him off on Elliot, who pawns him off on Todd and Turk. Later, they feel guilty about becoming insensitive towards him. They decide to treat his pain aggressively by recruiting Dr. Cox, Dr. Kelso, Jordan, and Dr. Wen to cheat the system and have Bober's gallbladder removed. ("My Last Day") He returns with advanced dementia. He was given the nickname Pickles, as that is the only word that he can say due to his illness. ("My Brother, Where Art Thou?")

Mr. Bober may be a reference to televsion history. Frances Bober was the maiden name of the wife of Sheldon Leonard. Leonard was the producer of The Danny Thomas Show (1953–64), The Andy Griffith Show (1960–68), The Dick Van Dyke Show (1961–66), and I Spy (1965–68).
